ABSTRACT

PURPOSE: To compare two combined surgical techniques, endoscopic endonasal surgery with transconjunctival reconstruction and transcaruncular surgery with transconjunctival reconstruction, when used to treat both medial and inferior orbital wall fractures. METHODS: A retrospective review of 63 patients who were followed up from January 2011 to December 2014 at Inha University Hospital for surgical reconstruction of combined medial and inferior orbital wall fractures was undertaken. We compared between the patients the computed tomographic scans, diplopia, extraocular muscle (EOM) movements, and Hertel's exophthalmometer exams pre- and post-6 months surgery. A total of 29 patients received endoscopic transnasal surgery with trasconjunctival reconstruction, and 34 received transcaruncular surgery with trans-conjunctival reconstruction. RESULTS: There were no significant differences between the two combined methods in terms of the primary and peripheral gaze diplopia or the restriction of EOM movement 6 months after surgery. However, statistically significant differences were observed in exophthalmometer measurements 6 months after surgery. CONCLUSIONS: The two combined surgical methods showed similar results in terms of postoperative primary and peripheral gaze diplopia, EOM restriction, and enophthalmos. With respect to postoperative peripheral diplopia, endoscopic endosnasal surgery with transconjunctival reconstruction showed several advantages over the other method considered in this study. An appropriate surgical method should be selected by comparing the relative advantages and disadvantages.

ABSTRACT

Hepatocyte growth factor (HGF) and its receptor, cMET, play critical roles in cell proliferation, angiogenesis and invasion in a wide variety of cancers. We therefore examined the anti-tumor activity of the humanized monoclonal anti-HGF antibody, YYB-101, in nude mice bearing human glioblastoma xenografts as a single agent or in combination with temozolomide. HGF neutralization, The extracellular signal-related kinases 1 and 2 (ERK1/2) phosphorylation, and HGF-induced scattering were assessed in HGF-expressing cell lines treated with YYB-101. To support clinical development, we also evaluated the preclinical pharmacokinetics and toxicokinetics in cynomolgus monkeys, and human and cynomolgus monkey tissue was stained with YYB-101 to test tissue cross-reactivity. We found that YYB-101 inhibited cMET activation in vitro and suppressed tumor growth in the orthotopic mouse model of human glioblastoma. Combination treatment with YYB-101 and temozolomide decreased tumor growth and increased overall survival compared with the effects of either agent alone. Five cancer-related genes (TMEM119, FST, RSPO3, ROS1 and NBL1) were overexpressed in YYB-101-treated mice that showed tumor regrowth. In the tissue cross-reactivity assay, critical cross-reactivity was not observed. The terminal elimination half-life was 21.7 days. Taken together, the in vitro and in vivo data demonstrated the anti-tumor efficacy of YYB-101, which appeared to be mediated by blocking the HGF/cMET interaction. The preclinical pharmacokinetics, toxicokinetics and tissue cross-reactivity data support the clinical development of YYB-101 for advanced cancer.

ABSTRACT

Anal incontinence following pelvic trauma, surgery, or neurologic disorders has significant medical and social implication. Both Chetwood in 1902 and Bistom in 1944 utilized coccygeal origin portion of the gluteus maximus muscle for the anal sphincter reconstruction. The gluteus maximus muscle is a broad, fan-shaped muscle with a wide origin from the ilium sacrum and coccyx and a narrow insertion along the iliotibial band of the lateral femur. Its blood supply is from the inferior gluteal artery and its innervation is from L-5, S-1 roots by means of the inferior gluteal nerve Incisions are placed at the inferior border of the ischial tuberosity. Subcutaneous tunnels are created about the rectum and gluteal and perirectal incisions. Two overacting slings are being created Their opposing pull creates sphincter or valve effect about the distal rectum. We have experienced 2 cases of irregular, deep soft tissue defects of the perianal region requiring muscle coverage with the gluteus maximus muscle overlapping slings. We think the use of the gluteus maximus muscle is one of the most useful method for reconstruction of the anal sphincter mechanism.

ABSTRACT

Pterygium is a common disease recurred frequently. Although several methods have been suggested for prevention of recurrences there was no royal road for that. In this study, we applied argon laser on each pterygium of 55 eyes and evaluated the effect of the argon laser treatment by observing the recurrence rate. The stage of pterygium was evaluated by the degree of corneal involvement and the number of blood vessels frorn the base to apex of the pterygium was also counted. We applied argon laser to the pterygium before or after surgical removal. The recurrence rate of pterygium was high in the persons under 50 years old and having many number of fibrovascular blood vessels in the pterygoid mass. This difference was statistically significant(p<0.05). However, sex of patients and size of pterygium did not influence the recurrence rate. Conclusively, argon laser treatment for pterygium would be recommendable to the aged patients having a few number of blood vessels in pterygoid mass.

ABSTRACT

The position of the upper lid crease is important factor in the reconstruction for lid anomaly and cosmetic defect. We were determined the proportions of the upper lid contour based on the following measurements in 115 children with bilateral lid crease and divided into 4 groups according to patient's eye(group I: below 4 years, group II: 5 ~ 7 years. group III: 8 ~ 10 years. group IV: 11 ~ 15 years); lashline-lid crease(LC), lashline-lower brow(LB), vertical fissure(VF), horizontal fissure(HF). All measured values(LC, LB, VF, HF) were increased with age(p<0.05). VF/HF and LC/LB ratio were not statistically significantly different between each age group. We concluded that when eye open, normal upper eyelid crease position was less than one fourth in I, II group. but approximately one fourth in III, IV group, and when eye closed, upper eyelid crease position is less than one third in I, II group but about one third in III, IV group.
